| To all ladies married recently... I need to have my married name added to my passport. Now, I got all the info from the Passport Canada website regarding the fee and what documentation they need... but I'm unclear on one thing. Do I need to complete a whole new passport application form, including a guarantor's signature and new pictures? Or am I able to just go in/mail the documentation they specify on their website and request the change without a whole new form? So confusing....
